Casa Pacifica
Casa Pacifica is a Transitional Residential Full Service Partnership program designed to provide comprehensive, psycho-social rehabilitation and recovery for voluntary adults with chronic and persistent mental illness and dual diagnosis. The program provides a schedule of daily classes to promote recovery and independent living. Services also include Case Management for the Public Conservator's Office, payeeship, individual therapy, medication support, psychiatry, vocational services, and peer support. Clients are welcomed into a warm, homelike atmosphere where emphasis is placed on identifying individual motivating factors and goals toward recovery.
Graduates of the Casa Pacifica residential program will be encouraged to join our Alumni program. This program is designed to continue the process of recovery in the community and further the transition to independent living. The program includes transitional case management, where clients are visited in their residence and continually taught basic independent living skills. Assistance is also provided with housing plan & maintaining residence. We also provide alumni events to promote social connections.

Program Goals
* To work closely in partnership with clients to help them learn or relearn the basic independent living and coping skills necessary to excel and enjoy meaningful independent living.
* To support the client in developing a supportive social network and linking them with community support once clients graduate and move into the alumni program.
* To assist clients in completing a Wellness Recovery Action Plan to reduce or eliminate hospitalization and increase crisis management skills.
* To assist and support clients in achieving their vocational goals.
Admission Criteria
* 18 years of age or older and voluntary
* Be free from non-psychiatric medical conditions requiring more than outpatient care.
* Not a current imminent danger to self or others.
* Have a chronic and persistent mental illness.
* Be willing to participate in program activities consistent with their Individual Service Plan goals such as taking medications as prescribed, independent living skills and/or vocational training and development.
* Funding through SSI/ SSDI with Medi-Cal. Rent fees are consistent with standard board and care rates.
Referrals
Referrals can be initiated by contacting the Program via phone/fax. The applicant will be invited to visit the program for a tour to meet the staff and clients. Interested candidates will be interviewed to mutually determine if Casa Pacifica can meet your needs.
Location
Casa Pacifica is located in a spacious 14-bed facility in a predominantly residential neighborhood, just one block east of the beach and a city park, and one block west of the main street in Oceanside. It is approximately ½ mile west of Interstate 5.
